People living with this condition are usually very aware of the need to control their blood sugar, diet, exercise and cardiovascular and kidney health. However, there is a hugely important microvascular complication that often does not receive the preventive attention it demands: diabetic retinopathy.

This eye condition is the leading cause of irreversible blindness among working-age adults in developed countries. Most alarming from a medical point of view is that almost all severe cases of sight loss due to diabetes could be avoided with early diagnosis and appropriate follow-up. At Clínica Ramírez & Gálvez, we explain how high blood sugar affects the inside of your eyes and why fundus examinations are non-negotiable if you have diabetes.

How does diabetes damage the retina? 🧐

The retina is the layer of light-sensitive tissue lining the back of the eye, acting as the “photographic film” that captures images and sends them to the brain. To work properly, the retina needs a constant supply of oxygen and nutrients through a dense, delicate network of microscopic blood vessels.

When blood glucose levels remain high over a long period, the walls of these small vessels weaken and their structure is profoundly altered:

Exudation and fluid leakage (Non-Proliferative Diabetic Retinopathy): The damaged vessels lose their natural seal and begin to leak serum, fat and small haemorrhages into the retinal tissue. If this fluid builds up in the central area of the retina (the macula, responsible for detailed vision and reading), Diabetic Macular Oedema develops – the most common cause of sight loss in diabetic patients.

Lack of blood supply and abnormal growth (Proliferative Diabetic Retinopathy): Over time, many capillaries become completely blocked and stop carrying blood to areas of the retina (ischaemia). In response to this lack of oxygen, the eye tries to defend itself by releasing growth factors to create new blood vessels (neovessels). However, these new vessels are fragile, grow in a disorderly way and tend to break easily, causing massive bleeding inside the eye (vitreous haemorrhage), tractional retinal detachment or neovascular glaucoma.

The big trap: a completely silent disease 💡

The greatest danger of diabetic retinopathy is how treacherously it progresses. In its early and intermediate stages, vascular damage gradually accumulates in the periphery of the retina without causing any symptoms at all: no pain, no redness and no noticeable loss of sharpness.

A diabetic patient can have 100% visual acuity while simultaneously developing microaneurysms, exudates and areas of severe ischaemia in the retina. When the patient starts to notice blurred vision, dark spots or distortion of straight lines, it means the disease has already reached the macula or an advanced complication has occurred. Waiting until you notice visual symptoms before seeing an ophthalmologist means arriving too late for prevention.

Precision diagnosis: more than just an eye test 🔬

A routine refraction test to update your glasses cannot assess the state of a diabetic retina. To detect the impact of sugar on the deep layers of the eye, advanced diagnostic tests are essential:

Dilated fundus examination: We apply drops to widen the pupil and directly examine, by ophthalmoscopy, the state of the blood vessels, the optic disc and the retinal periphery.

OCT (Optical Coherence Tomography): This is the gold-standard test for diagnosing macular oedema. It performs a microscopic cross-sectional “scan” of the macula with micrometric precision, detecting fluid build-up invisible to direct examination before it destroys the photoreceptor cells.

OCT Angiography: This allows us to analyse the capillary map of the retina without injecting intravenous contrast, quickly and safely identifying areas lacking blood supply.

Modern treatments: stopping progression in time 🩺

Fortunately, the management of diabetic retinopathy has undergone a revolution over the last decade. The cornerstone will always be strict control of blood sugar and blood pressure together with your endocrinologist or GP, but at eye level we have highly effective therapies:

Intravitreal injections (anti-VEGF drugs and corticosteroids): These are given painlessly, directly into the eye, in the consulting room under local anaesthetic. They “halt” fluid leakage, reabsorb macular oedema and seal abnormal vessels, restoring sharp vision in a large proportion of patients.

Retinal laser photocoagulation: Used to treat areas of ischaemia and prevent the appearance of neovessels in proliferative forms of the disease.

Vitrectomy: Micro-incision surgery reserved for very advanced cases with internal bleeding or retinal detachment.

Caring for your retinal health in Lucena and Torre del Mar 🏥

If you have diabetes (whether Type 1 or Type 2), international medical guidance is clear: you should have at least one complete fundus eye examination a year, even if you feel your vision is perfect.

At both our Lucena and Torre del Mar facilities, our team of ophthalmic surgeons has the cutting-edge diagnostic technology needed to monitor the health of your retina with the utmost rigour and design a preventive plan tailored to you.

Protecting your eyes from diabetes is a decision in your hands today that safeguards your independence and quality of life tomorrow.

Do you have diabetes and has it been more than a year since your fundus or macula was examined?

Don’t put your vision at risk. Book your retina check-up at our clinics in Lucena or Torre del Mar and look after your eyes with true experts! 📅✨

Diabetes mellitus is one of the most prevalent chronic diseases in today’s society.
