They move when you move your eyes and seem to dart away when you try to look at them directly. These are the well-known “floaters” or, in medical terms, myodesopsia. Although they can be very annoying, in most cases they are harmless.

However, they can sometimes be the warning sign of a serious problem. At Clínica Ramírez & Gálvez, we want to give you the peace of mind of knowing how to tell them apart. We explain why they appear and, most importantly, when you should seek emergency care.

What exactly are floaters and why do they appear? 🤔

Our eye is filled with a clear, gel-like substance called the vitreous humour. In youth, this gel is uniform and dense. Over the years, the vitreous tends to liquefy and shrink. In this process, the collagen fibres it contains clump together.

These clumps float inside the eye and cast their shadow onto the retina. Those shadows are what we perceive as floaters. It is a natural part of the eye’s ageing process and is especially common in short-sighted people or those who have had cataract surgery.

Vitreous detachment: the most common cause of their sudden appearance

The most frequent reason a patient suddenly starts seeing floaters is a Posterior Vitreous Detachment (PVD). This happens when the vitreous gel, as it shrinks, separates from the back wall of the eye (the retina). It is a very common event from the age of 50–60.

In the vast majority of cases (around 90%), a PVD is a benign process that is not serious. Over time, the brain usually “learns” to ignore these new spots.

The warning signs: when should you seek emergency care IMMEDIATELY? 🚨

The real danger of a vitreous detachment is that, as it separates, the gel “pulls” on the retina and tears it, causing a retinal tear. If this tear is not treated in time with laser, it can progress to a retinal detachment, a very serious condition that can cause blindness.

That is why you should see an emergency ophthalmologist without delay if you experience any of these three symptoms:

SUDDEN appearance of MANY floaters: Not one or two, but a sudden “shower” of black dots.

Seeing flashes of light (photopsia): Seeing bright flashes or “lightning streaks”, especially in your peripheral vision, even with your eyes closed. This indicates that the vitreous is pulling on the retina.

Appearance of a “curtain” or dark shadow: If you notice a fixed dark patch covering part of your field of vision (like a curtain falling), it is a sign of the utmost urgency, as it could indicate that the retina has already detached.

Know to prevent, act to cure 🌟

Most floaters are a normal part of our eyes’ ageing. However, it is essential not to ignore the warning signs. A sudden appearance of many floaters, especially if accompanied by flashes of light, warrants an urgent visit to the ophthalmologist for a retinal (fundus) examination.

Detecting a tear in time allows it to be sealed with laser during a simple consultation, avoiding complex surgery. If in doubt, prevention is always your best ally.
