A procedure in which we use a CO₂ laser to make the incisions and to treat excess skin and fat bags.
The CO2 laser seals small blood and lymphatic vessels instantly as it cuts.
Minimal bleeding, which dramatically reduces bruising and post-operative swelling.
The thermal effect of the laser stimulates natural collagen production in the skin, improving its texture.
CO2 laser blepharoplasty is a surgical procedure that uses a high-energy laser beam instead of a conventional scalpel to make the incisions.
🔹 It cuts with precision: the laser vaporises tissue in a controlled way, allowing the surgeon to make clean cuts of unrivalled finesse.
🔹 It coagulates instantly: as it cuts, the heat of the laser instantly seals small blood and lymphatic vessels.
This means surgery with minimal bleeding, giving the surgeon a perfect view of the anatomy and increasing the safety and precision of the procedure.
This is the big difference, and the reason why at Clínica Ramírez & Gálvez we have chosen this technology.
🔹 Less bleeding and bruising: because the blood vessels are cauterised instantly, there is practically no bleeding. This dramatically reduces the troublesome bruising that can follow surgery.
🔹 Faster recovery: with less bleeding and less swelling (the laser seals the lymphatic vessels), recovery is much more comfortable and less painful, and you can return to your social life sooner.
🔹 Collagen stimulation (a double benefit): the thermal effect of the laser on the skin stimulates natural collagen production. This means that, as well as treating the bags and the skin, we improve the texture and firmness of the eyelid skin in the long term.
🔹 Maximum surgical precision: the laser gives the oculoplastic surgeon superior control to sculpt the tissues and calculate the exact amount of skin and fat to treat.
🔹 Option of resurfacing during the same procedure: the same CO2 laser can be used in fractional mode (resurfacing) during surgery to treat fine lines ("crow's feet") and improve the overall quality of the skin.
CO2 laser blepharoplasty is an excellent option for most patients seeking aesthetic rejuvenation of the eyes. It is ideal if you have:
🔹 Excess skin on the upper eyelids (a tired or hooded look).
🔹 Fat bags on the lower eyelids.
🔹 Fine lines around the eyes (which can be treated with complementary resurfacing).
🔹 A wish for a faster recovery with less bruising.
In addition, the laser is the tool of choice for transconjunctival lower blepharoplasty, an advanced technique that allows us to remove fat bags through the inner surface of the eyelid, thus avoiding any visible scar on the skin.
Because the laser is less traumatic to the tissues, recovery is easier.
🔹 Swelling and discomfort: it is normal to experience some swelling and mild discomfort, which are significantly less than with the traditional technique. Applying cold compresses will be your best ally during the first 48 hours.
🔹 Stitches: although the laser incision heals excellently, tissue cut with a laser heals slightly more slowly than tissue cut with a scalpel. For safety, the stitches may be kept in for one or two days longer than usual.
🔹 Back to normal: the initial recovery usually takes 7 to 10 days. You should protect the area from the sun with sunglasses and avoid strenuous exercise for the first 2–3 weeks.
Find out in this video how blepharoplasty not only improves appearance but can also resolve functional problems of the eyelids.
Dr. Javier Ramírez explains, with a real case, how this treatment can relieve discomfort and improve our patients' vision, as well as rejuvenating their eyes.

There are different types of blepharoplasty depending on the area to be treated and the specific needs of each patient.
For comprehensive and harmonious aesthetic rejuvenation, a combined procedure was planned: a brow lift to raise the frame of the eyes, an upper blepharoplasty to open up the eyelid, and a lower blepharoplasty to correct the bags. The result is a visibly more open, fresh and rested expression, in complete harmony with the patient's natural features.
To correct the feeling of "heavy eyelids" and rejuvenate the eyes, a high-precision procedure was performed combining three techniques in one: an upper blepharoplasty to remove excess skin, repositioning of the lacrimal gland, which had dropped, and a brow lift (browpexy). The result, just a month and a half later, is a visibly more open, clearer and more youthful look, with an even eyelid and a higher brow.
In this case, as well as excess skin on the upper eyelid, the patient had a bulge in the outer area that became much more noticeable when she closed her eyes. This "bulge" was caused by prolapse (or herniation) of the lacrimal gland. For an optimal aesthetic result, an upper blepharoplasty was performed, combining removal of the excess skin with repositioning and anchoring of the gland in its correct position, thus treating the problem at its root.
This case is the perfect example of comprehensive aesthetic rejuvenation of the eyes, in which all the signs of ageing have been treated for a harmonious result. The surgery combined a brow lift with a full blepharoplasty (upper and lower) to reposition the tissues and remove excess skin and bags. As a complementary treatment, CO₂ laser resurfacing was carried out to improve the quality and texture of the skin, achieving a complete yet always natural transformation.
The challenge in this young patient was to correct the "A-frame deformity" of the eyelid, which gave her a hollow, uneven appearance. A high-precision upper blepharoplasty was planned, removing a minimal amount of skin and focusing on restoring volume to achieve an ideal eyelid contour. The result is a much more symmetrical, harmonious and aesthetically balanced look, preserving her natural appearance as much as possible.
To rejuvenate this patient's eyes, a two-fold plan was designed. First, the fat bags were removed by transconjunctival blepharoplasty (from the inside, with no scar on the skin). Second, lateral skin tightening was performed to firm the eyelid skin. This combination of techniques allows us to achieve a firmer, more rested and natural look, always respecting the original expression.

We answer the most common questions about CO2 laser blepharoplasty. Here you will find all the information you need to make your decision with complete confidence.
The main advantage is minimal bleeding. The CO2 laser instantly coagulates and seals small blood and lymphatic vessels. This means a recovery with less bruising and swelling, allowing a more comfortable and faster recovery. In addition, the laser stimulates collagen production, improving skin texture.
Blepharoplasty is designed so that scars are practically imperceptible.
On the upper eyelid: the incision is hidden in the natural crease of the eyelid.
On the lower eyelid: the CO2 laser is the ideal tool for the transconjunctival technique, that is, from the inside of the eyelid. In this way, we can remove the fat bags without leaving any visible scar on the skin. If skin needs to be removed, the incision is made just below the lash line, where it remains camouflaged.
No. An artificial result does not depend on the tool (laser or scalpel) but on poor technique. The aim of modern blepharoplasty is not to change your expression but to aesthetically rejuvenate the eyes, giving them a fresher, more rested and brighter appearance. That is why it is essential to see an oculoplastic surgeon, the specialist who best knows the anatomy of the eyelids, to achieve a natural result. In fact, the precision of the laser helps the surgeon to be more exact.
Because the laser causes less trauma to the tissue, the initial recovery is usually faster, with less swelling and bruising. Most patients return to their social life within 7–10 days. Make-up and contact lenses are generally allowed once the stitches (if any) have been removed. Although initial results are visible early on, the final result and the improvement in the skin settle over the following months.
The surgery is not painful. It is usually performed as an outpatient procedure under local anaesthesia with deep sedation (administered by an anaesthetist). The patient is relaxed, feels nothing during the procedure and can go home the same day.
No. As well as the aesthetic benefit, for many patients it is functional surgery. Excess skin on the upper eyelid can “droop” over the lashes and act like a visor, reducing the field of vision. By removing this skin, patients not only look aesthetically rejuvenated but also regain quality of vision and feel less heaviness around the eyes.
