What is lower eyelid blepharoplasty?
Lower eyelid blepharoplasty is a surgical treatment performed to remove the fat bags that build up under the eyes, as well as the excess skin that contributes to a tired, aged appearance.
Benefits of lower blepharoplasty:
✅ Removal of bags and dark circles.
✅ Improved appearance of the eye area.
✅ A less tired or aged look.
✅ A minimally invasive procedure with a quick recovery.
When is lower blepharoplasty surgery needed?
The need for lower blepharoplasty varies from patient to patient and depends on how much the appearance of their eyelids affects their image and confidence. Not every case requires surgery, but it is a recommended option when signs of ageing or certain inherited features significantly affect the appearance of the face.
When do we recommend lower blepharoplasty?
🔹 Visible fat bags under the eyes that give a tired appearance.
🔹 Deep dark circles that accentuate facial tiredness.
🔹 Sagging and excess skin in the lower eye area that gives an aged look.
🔹 When it affects the patient's image and self-esteem
Factors to consider before surgery:
- Every case is unique. It is a highly personalised operation, and we explain in detail which technique is most suitable for each patient.
- Realistic expectations: It is important to understand that surgery improves appearance but does not stop natural ageing.
Why do bags appear on the lower eyelids?
Bags on the lower eyelids develop through a combination of genetic factors and the natural ageing process. Some of the factors that influence their appearance include:
✅ Age: Over time, the tissues that support the skin and the fat pads in the lower eyelids weaken, causing them to sag.
✅ Genetics: Many people have an inherited tendency to develop eye bags at an early age.
✅ Sun exposure: UV radiation speeds up skin ageing and can contribute to sagging.
✅ Lack of sleep: Accumulated fatigue and lack of rest can make eye bags worse.
✅ Smoking and alcohol: Both damage skin health, contributing to premature ageing.
Restore your look with safe, effective surgery
Lower eyelid blepharoplasty is performed under local anaesthesia and sedation to ensure the patient's comfort. There are several types of lower blepharoplasty, depending on the technique used and the patient's needs.
- Transconjunctival blepharoplasty
- It is performed through a small incision on the inner surface of the eyelid (conjunctiva).
- It leaves no visible scars.
- It is mainly used in younger patients with excess fat in the eyelids but no skin laxity.
- Faster recovery and a lower risk of complications.
- External or subciliary blepharoplasty
- An incision is made just below the lower lash line.
- It allows both excess fat and surplus skin to be removed.
- It is recommended for people with skin laxity or wrinkles in the lower eyelid area.
- It leaves a very fine scar that becomes imperceptible over time.
- Blepharoplasty with fat repositioning
- Instead of removing the fat from the bags, it is redistributed to avoid a hollow look in the area.
- Ideal for people who have lost volume around the eyes.
- Lower blepharoplasty with CO2 laser
- The fat bags are removed through a transconjunctival approach (from inside the eyelid); the laser vaporises the excess fat without the need for external incisions.
- Skin rejuvenation: the heat of the laser stimulates collagen production, tightening the skin and reducing fine lines.
- Faster recovery.
Recovery:
Depending on the surgical technique, recovery may be longer or shorter, but swelling and bruising are normal in the first few days and disappear within one to two weeks. The final results can be seen after about a month, once the area has settled.
Before and after
Upper and Lower Blepharoplasty and Brow Lift
For a comprehensive, balanced aesthetic rejuvenation, a combined operation was planned: a brow lift to raise the frame of the eyes, an upper blepharoplasty to open up the eyelid, and a lower blepharoplasty to correct the bags. The result is a visibly more open, fresh and rested expression, in complete harmony with the patient's natural features.
Before and after
Blepharoplasty and Lacrimal Gland Repositioning
In this case, in addition to excess skin on the upper eyelid, the patient had a bulge in the outer area that became much more noticeable when she closed her eyes. This "bulge" was caused by luxation (or herniation) of the lacrimal gland. For the best aesthetic result, an upper blepharoplasty was performed, combining removal of the surplus skin with repositioning and anchoring of the gland in its correct position, treating the problem at its root.
Before and after
Upper and Lower Blepharoplasty, Brow Lift and CO2 Laser
This case is the perfect example of a comprehensive aesthetic rejuvenation of the eye area, in which all the signs of ageing were treated for a balanced result. The planned operation combined a brow lift with a full blepharoplasty (upper and lower) to reposition the tissues and remove excess skin and bags. As a complementary treatment, CO₂ laser resurfacing was carried out to improve the quality and texture of the skin, achieving a complete yet always natural transformation.

Answers to your questions
We answer the most common questions about the symptoms and treatment of excess skin on the lower eyelids. Here you will find all the information you need to make your decision with complete confidence.
Is lower eyelid blepharoplasty painful?
The procedure is performed under local anaesthesia, so you will not feel pain during surgery. Afterwards, some swelling and discomfort are normal and are controlled with painkillers.
How long does the surgery take?
The procedure usually takes between 45 and 60 minutes, depending on the complexity of the case.
Will I need to stay in hospital after surgery?
No, lower eyelid surgery is a day-case procedure, so you can go home the same day.
Can I wear make-up immediately after surgery?
It is not advisable to apply make-up around the eyes for at least a week after surgery, to avoid infection and allow the wounds to heal properly.
How long does it take to heal?
The scars fade quickly and, in most cases, are invisible. The full healing process can take between 3 and 6 weeks.
Are the results permanent?
Yes, the results of lower eyelid blepharoplasty are long-lasting, although the ageing process will continue and new signs of laxity may appear over time.
