“Doctor, I want to get rid of these dark circles.” It is one of the phrases we hear most often in our clinic. However, in 80% of cases, when the patient points to their lower eyelid, they are not referring to a dark circle, but to an eye bag. And this confusion is the main reason why so many creams and treatments fail: the wrong solution is being applied to a misdiagnosed problem.
At Clínica Ramírez & Gálvez, we believe a good diagnosis is everything. Today we are going to clear up, once and for all, the difference between an “eye bag” and a “dark circle”, why they appear and, most importantly, why their treatment is radically different.
What Is an EYE BAG? (A Volume Problem) ⛰️
An eye bag is, quite simply, a bulge. It is a problem of protruding volume.
- Why it appears: As we explained in our previous post, eye bags are not a sign of tiredness. They are a structural problem. We have pads of orbital fat held in place by a membrane (the septum). Over time, or due to genetics, this membrane weakens and the fat “herniates”, that is, it pushes forwards, creating that bulge.
- The Solution: If the problem is structural, the solution must be too. Eye bags cannot be removed with creams. The only definitive treatment is surgery (lower blepharoplasty) to remove or reposition that fat.
What Is a DARK CIRCLE? (A Colour or Hollowing Problem) 🎨
A dark circle, on the other hand, is not a bulge. A dark circle is either a hollow under the eye or a dark colour.
- Hollow Dark Circle (Tear Trough): This is the most common type. It is a depression or “valley” that appears between the lower eyelid and the cheek. It is caused by the loss of bone and fat volume with age, or by facial anatomy itself. It gives a “shadowed”, tired appearance.
- Pigmented Dark Circle (Brown): This is a darkening of the skin due to excess melanin. It is genetic in origin and more common in darker skin types.
- Vascular Dark Circle (Purple/Blue): This occurs in people with very thin skin, through which the blood vessels and the orbicularis muscle underneath show.
The Solution to Each Problem (The Fatal Mistake of Confusing Them) 💡
This is the key to everything. The treatment for one does not work for the other:
- If you have EYE BAGS (Bulge): The solution is surgical.
- Lower Blepharoplasty: The fat is removed (often via a transconjunctival approach, with no scar on the skin) to smooth the eyelid.
- If you have DARK CIRCLES (Hollowing): The solution is to fill.
- Hyaluronic Acid Filler: A specific hyaluronic acid is injected into the tear trough to level the surface and eliminate the shadow.
- If you have DARK CIRCLES (Colour): The solution is to treat skin quality.
The biggest mistake is trying to treat an eye bag (a bulge of fat) with hyaluronic acid filler. Not only will this fail to disguise it, it will create an even bigger bulge and may cause the dreaded “Tyndall effect” (a bluish tint).
An Expert Diagnosis Makes the Difference 🌟
In most patients, the problem is mixed: there is an eye bag with a hollow (dark circle) next to it. That is why an expert assessment is essential. An oculoplastic specialist will be able to diagnose exactly which component is the problem and design a personalised plan (perhaps surgery and, a few months later, a touch of filler) for a natural result.
